RFCs
A normative change reaches the specification only through an RFC. This is every one that has been opened, with its status and its decision.

What needs one
Any change to a schema field, cardinality, enum, required or optional status, conformance level, or the meaning of a defined term. Plus governance and licensing. Everything else — typos, examples, documentation, tests, website copy — takes the fast path and needs one approval.
Who may open one
Anyone. You do not have to be a maintainer and you do not have to have written any code. Whoever has run into a problem implementing the specification is often best placed to describe what is wrong with it.
